Printed from your driver education course after completing Lesson 1 and the written test. Print in color if it has a green background; newer black-and-white certificates can be printed in B&W.

Your receipt number (starts with 452PT) from TDLR showing you purchased the packet.

Original or certified copy. Hospital birth records are not accepted.

Original card with your full 9-digit number. Laminated cards are not accepted.

Utility bill, bank statement, or other document showing name and Texas address.

Must be from a different source than #1. Dated within 90 days.

Required if under 18 without a diploma or GED. Signed by your school. Homeschool students: parent signs as administrator.

Application for minor's license. Both student and parent must sign.

Valid Texas driver's license of the parent signing the DL-14B.

Cash, check, or card accepted at most DPS offices.
