Texas PTDE Forms & Documents

Need help with paperwork? Click any form below for a step-by-step guide on how to fill it out correctly.

Keep Your Forms Safe

Download and save these forms to your computer. You'll need them at multiple points during the PTDE process. Lost forms can delay your teen's progress significantly.

Most Important Form

DL-14B Driver License Application

The main application form for both learner permits and full licenses. Fill this out at home to save 20 minutes at the DPS.

Driving Logs (DL-91B & 30-Hour Log)

Track your 14 hours of in-car instruction (DL-91B) and 30 hours of practice driving. Understand the requirements for each log.

VOE (Verification of Enrollment)

Proof that your teen is enrolled in school. Learn when to get it, how long it's valid, and what to do during summer.

Usually obtained from your school — homeschool families can download and complete it themselves

Texas Residency Affidavit

For teens who don't have utility bills in their name. Learn what supporting documents you need.

Texas Driver Handbook (DL-7)

The official Texas driving handbook from DPS. Covers traffic laws, road signs, and rules of the road — useful for studying before the written test.
