Section 1: Personal Information

Full Legal Name

Use your name exactly as it appears on your birth certificate or passport. No nicknames.

Date of Birth

Use MM/DD/YYYY format.

Sex

Mark M or F as shown on your identity documents.

Social Security Number

Required. If you don't have one, bring documentation to the DPS explaining why.

Section 2: Physical Description

Height

Enter in feet and inches (e.g., 5'6").

Weight

Enter your current weight in pounds.

Eye Color

Common options: BLU, BRN, GRN, HAZ, GRY, BLK.

Hair Color

Common options: BLN, BRN, BLK, RED, GRY, WHI, BAL (bald).

Section 3: Address Information

Texas Residence Address

Physical home address in Texas. PO Boxes are not accepted for residence address.

Mailing Address

Can be different from residence. This is where your license will be mailed.

County of Residence

The Texas county where you live (e.g., Harris, Travis, Dallas).

Section 4: Yes/No Questions

U.S. Citizenship

If not a citizen, you'll need immigration documents.

Previous License History

List any state where you've held a license. Answer honestly—they will check.

Medical Conditions

Conditions that could affect safe driving. If unsure, answer "Yes" and explain at the office.

Criminal History

Certain convictions must be reported. When in doubt, disclose.

Section 5: Organ Donor

Organ Donor Registration

Optional. You can register to be an organ donor. This appears as a heart symbol on your license.

Applicant Signature

Sign in black or blue ink. For minors, a parent/guardian must also sign.

Parent/Guardian Signature

Required for applicants under 18. The signing adult accepts financial responsibility.

Common Mistakes to Avoid

  • Using nicknames instead of legal name

    Use your name exactly as it appears on your birth certificate.

  • Leaving the SSN blank

    SSN is required. If you don't have one, bring documentation explaining why.

  • Using a PO Box for residence address

    Use your physical home address. PO Boxes can only be used for mailing address.

  • Not signing the form

    Both the applicant AND parent/guardian (for minors) must sign before arriving.

  • Forgetting parent signature

    If under 18, the parent must sign to accept liability. Don't forget this!
